Chronic disease management

Chronic disease management is a new program introduced by the HSE for patients aged 18 and above with one of the following conditions:

  • Asthma

  • COPD

  • Diabetes Mellitus

  • Atrial Fibrillation

  • Ischaemic Heart Disease including Heart Attacks and Angina

  • Stroke

The patient will be seen twice a year initially by the nurse who will take bloods check measurements including blood pressure and organise for a review either in person or by telephone by a doctor. This is an opportunity to discuss all aspects of your chronic disease and hopefully optimise management including prescriptions. A six month prescription will be generated at this appointment and sent to your pharmacy and a new appointment made in six months to be reviewed by the nurse again. A further doctor’s review will follow and an appointment made for a further six months. This will continue indefinitely and should result in significant improvement in the management of your chronic disease and also hopefully reduce the need for further attendances to the doctor and also hospitalisations.

This program is completely free to the patient but is only open to those with either a full medical card or a doctors visit card. If you’re interested you can find further information from HSE at the link below :

You can make an appointment to see your doctor who will then enrol you in the programme.

Currently there are no provisions for private patients to avail of the service but we would be happy to tailor a similar approach to anyone who is interested.
