Ordering your prescription....

A prescription request form can be collected from reception or you can email your prescription request to appointments@clannadmedicalcentre.ie

Please provide your Name, Date of birth, list of Medication and the paharmacy you would like us to send it to.

We provide a print out on request where there are multiple medications, please ask the practice manager.

**You must list each medication you need**

Please allow 3 working days before your prescription is sent to your pharmacy.

It is your responsibility to ensure your prescriptions are renewed when the last script is collected from your pharmacy. Ordering in good time will reduce the risk of you running out of your medication.  

Repeat Prescriptions
The charges for repeat private prescriptions are €20 for 6 monthly repeat prescriptions, €10 for 3 monthly and €5 for monthly prescriptions. Prepayment for this service will be requested by SMS payment link.
