Sick Certs and Social Welfare Certs

From March 1st 2019 all weekly certs are being replaced with a closed certification process. The Dept of Social Protection have given guidelines for all GPs to allow a specific time off for specific illness.

This means you will see your GP for the initial assessment, collect your sick note, social welfare booklet and cert to cover the expected period of illness. We no longer issue weekly sick notes or weekly sick social certifcates.

After your last day of sickness, you will return to work as agreed, without any need to see the Dr further - unless you need a review. In this case an appointment with your Dr should be booked by you, before the return to work date, to discuss any further time off.

Making an appointment or ordering a prescription…

You can beat the phone queue by emailing us at appointments@clannadmedicalcentre.ie 

For appointments include your name, date of birth and tel no to request the appointment you need.

For prescription requests...........Please allow 3 working days before collection. It is your responsibility to ensure your prescriptions are renewed when the last script is collected from your pharmacy. Ordering in good time will reduce the risk of you running out of your medication.

The charges for repeat private prescriptions are €20 for 6 monthly repeat prescriptions, €10 for 3 monthly and €3.33 for monthly prescriptions. You can pay for your prescription at reception.
